If you’ve lost a loved one to a workplace incident, we invite you to honour them with a plate in the Workers’ Commemorative Park. This Park is a space where memories live on, stories are acknowledged, and lives are never forgotten.

To begin the process, please complete the form on this page. Whether you’re a family member, friend, or colleague, we’ll work with you to ensure your loved one is remembered with dignity and care.

The Park has been designed to include dedicated plates as part of its memorial landscape. These plates provide a permanent and personal way to pay tribute. There is no cost to families for their installation. 

We also welcome the sharing of stories, photos, or reflections to contribute to future commemorative events and educational materials. Your loved one’s life can continue to spark change and inspire safer workplaces for all.
